Attention All Applicants and
Cardiovascular Technology Students

Please be aware that students accepted to the Health Professions Programs (Nursing, RT, CVT, Ortho Tech, OTA) at Grossmont College are required to undergo a background check and/or a urine drug screening test before starting the program or being placed at a clinical site. We expect the cost of these procedures may be $70 - $120. The student will be responsible for paying these fees.

It should be noted that failure to pass either or both of these procedures may interfere with clinical placement and/or acceptance of the student into his / her selected program.

  5. Medical Release Form - CVT Handbook pg. 3 - print, read & sign
  6. Consent Form for Release of Medical Records print, read & sign
  7. Immunization Record and Statement of Health Form
  8. Student Survey
  9. Information for students purchasing scrubs
  10. Annual Medical Clearance Form (Physical)
  11. Tuberculosis (PPD) Annual Form
  12. Flu Vaccine Consent**
  13. Flu Declination Form- a physician's statement documenting an adverse reaction or a statement from your pastor indicating refusal for religious reasons is required.
  14. CPR Class - Resource List
  15. Malpractice Insurance Information - HPSO
  16. 2nd Year Survey
  17. Re-entry Application (if applicable)
  18. Worker's Compensation Information - Print all 3 forms:
    Injury Illness/Accident Report
    Employee Claim Form
    Employer's Report of Occupational Injury

**Influenza update- posted 7/07/2016
For the 2016-2017 flu season, the Cardiovascular Technology Program will no longer accept FLU MIST (nasal spray) per new Federal Advisory Committee on Immunization Practices (ACIP) and the Center for Disease Control (CDC).  Students must have the influenza vaccination which is offered through Grossmont College Health Services or a physician's office.
